Short text, long text, single choice, multiple choice, and agreement — the building blocks for any form you need to build.

Agreement / waiver fields

Add a waiver or terms block with a required confirmation checkbox. Volunteers must tick "I agree and confirm" before they can submit.

Create as many forms as you need — a waiver, a medical form, a role preferences questionnaire. Each one is separate and independently publishable.

Lock after submit

For waivers and legal agreements, enable "lock after submit" so responses can't be changed once recorded.

Volunteers see a Forms tab on their event page. Required forms show a banner so nothing gets missed. Answers are saved and pre-filled on return visits.

Forms live on the same page as the volunteer's rota and documents — no separate link, no separate login. Everything in one place.

Banner for required forms

When a required form is published, a banner appears at the top of the volunteer's event page. It disappears once they've submitted.

Answers saved and pre-filled

Volunteers can return and update their answers at any time (unless the form is locked). Previous answers are shown so they don't start from scratch.

Clear completion status

The tab shows a completion count (e.g. "Forms 1/2") so volunteers know at a glance what's outstanding.

See every response in one place

The results page for each form shows every volunteer's answers side by side. Download a CSV to take the data wherever you need it.

One row per volunteer, one column per field. Submitted and pending responses shown together so you can see who still needs to complete the form.

Download all responses as a spreadsheet. Useful for sharing waiver records, passing medical info to first aid, or processing dietary requirements.

Common questions

Can volunteers edit their answers after submitting?

Yes, by default — they can return and update responses any time. If you need responses locked (e.g. a waiver), enable "lock after submit" on that form and answers become permanent once submitted.

When do volunteers see a form?

Only when it's published. Forms in draft are invisible to volunteers, so you can build and adjust at your own pace. Publishing is a single toggle on the form card.

Can I have different forms for different events?

Yes — forms are per event. Each event has its own independent set of forms, so a waiver for one event doesn't appear on another.

Is there a limit on the number of forms or fields?

No — create as many forms as you need, with as many fields as each form requires.

Can I see which volunteers haven't completed a required form?

Yes. The results page shows pending volunteers at the bottom of the table. The People page also shows form completion status alongside each volunteer.
